The Problem

The problem is clinicians—Nurse Practitioners and Registered Nurses in particular—are forced to conduct significant and frequent ‘fact finding’ efforts to find useful information about their patients. This takes away valuable time the clinicians could otherwise be allocating to patient care & collaborating with nephrologists.

To solve this problem, our team created Smartapps to empower users to provide better quality healthcare by surfacing relevant and timely information in an easy to use fashion.

The Patient Care Summary

The intent of the Patient Care Summary is to show important information & actions for the clinicians to interact with in their daily patient care workflow. It includes basic demographic information, clinical alerts, & leverages ADT data in real-time from the hospital.

Surfacing Detailed Information

In addition to surfacing top line demographic information alongside important clinical information, the Patient Care Summary includes a recent history of Labs and Medications. Taken together, the clinician is able to identify patterns in the patient's treatment & communicate any changes to the patient's medication to other teammates.

Setting the Interactive Stage

With the goal of staying true to its title, the Patient Care Summary leverages interactive elements to store clinic-focused secondary information. The overview section launches a sidebar that contains contact information for relevant teammates, the previous 3 months of patient contact, and an embedded PDF of the documentation of last patient contact.

Leveraging Novel Data

A critical and innovative information pipeline we leverage as part of this project is Admissions, Discharges, and Transfer (ADT) data. Generated in real time directly from the hospital, this information provides RNs and NPs with critical & timely information about their patients.
